This position is currently scheduled 35 hours per week, day shifts. We are recruiting four full-time, permanent Physiotherapists to support the 8 Long-Term Care Homes that the Niagara Region Operates.

Position Summary:

Salary Pending Review

Reporting to the Director Resident Care (DRC), the Physiotherapist is responsible to provide comprehensive individualized physical assessments and treatment to residents within Long Term Care Home.  This is done through the completion of assessments, analysis, administration of diagnostic tests and observations, individual treatment planning, ongoing evaluation of client progress and goals, and integration of treatment recommendations within a case planning model.  Provides consultation and acts as a professional resource to a diverse team.  Works according to all applicable standards, acts and governing bodies.

Responsibilities:

Clinical Evaluation of residents (35% of time)

  • Assessing residents, creating, and implementing individualized treatment plans based on analysis and clinical reasoning.
  • Developing resident-centered plans and monitor goals and treatment plans.
  • Complete full clinical documentation and input statistical information related to all client activities that meet departmental, provincial, and professional standards of practice and documentation policies.

Provides coaching, mentoring and daily direction to staff in the LTC home (35% of the time)

  • Provides daily direction to the Rehabilitation workers in the LTC home.
  • Provides technical expertise and direction.
  • Works collaboratively with all staff in the LTC home to plan, implement, train, advise, coach, and evaluate, on occupational therapy.
  • Acts as a mentor to staff as Subject Matter Expert.
  • Work alongside staff observing and assisting, providing in the moment coaching.
  • Addresses any staff concerns through ongoing performance management.
  • Works collaboratively with Management.
  • Provides regular communication for the staff, residents, and families to discuss strategies and supports.
  • Works with the leadership team to identify potential risks, and appropriate mitigation measures and precautions.
  • Regularly meets with interdisciplinary team and participates in debrief sessions.
  • Regularly attend and actively participate in staff and intake meetings, coordinate treatment plans with other team members.
  • Observes and assists with care, providing in the moment coaching and feedback.  Responsible for addressing any observed culpable behaviour of staff and consulting with managers on related investigations.

Program Management and documentation (30% of time)

  • Document assessment and treatment as per guidelines established by College of Physiotherapists.
  • Collaborating with inter-disciplinary team within LTC home to develop resident centered plans that provide active based therapy.
  • Provide in-house staff education sessions on topics such as body mechanics, rehab equipment and devices, beck/neck care, fall prevention and ergonomic set-up
  • Provide education to long term care staff, residents and family members in several areas including falls management, safe resident handling and mobility.
  • Comply with any relevant legislation, regulations, and mandatory guidelines.
  • Participate in the continuous quality improvement program, including measurable       outcomes, satisfaction feedback, and quality improvement planning.
  • Policy and Program Planning, Implementation and Evaluation -Cooperate and participate in program evaluation and research.
  • Provide individual counselling and intervention to enable LTC clients to identify strengths and problems, establish goals and plans to reach them.
  • Attends case conferences regarding their assigned or referred cases or LTCH’s
  • Implement and evaluate the effectiveness of the care plans and make changes as required.
  • Plan, coordinate, manage & evaluate the resident's care plan working with the resident/substitute decision maker, physicians, nursing & interdisciplinary team, and contracted service providers/external resources
  • Participates in Interdisciplinary Care Conferences as needed.
  • Maintains timely, accurate and complete records, and assessments
  • Develops and sustains a plan of care to meet the assessed needs and interests of the individual resident
  • Documents in Point Click Care as required.
Requirements

Education:

  • Bachelors Degree in Physiotherapy. 
  • Master’s degree in Physiotherapy. 
  • Must be registered as a member of the Ontario College of Physiotherapists.

Knowledge:

  • Minimum of 4 years current, relevant experience in a LTC setting or similar setting.
  • Knowledge of and experience with Dementia Care and dealing with Responsive Behaviours an asset.
  • Knowledge in Gerontology, Alzheimer’s disease, related dementias an asset.
  • Demonstrated proof of mentoring, training, and coaching others
  • Point Click Care knowledge an asset

Other:

  • In accordance with the Corporate Policy and applicable Legislation/Regulation(s), the position requires the incumbent to undergo a Police Record Check and submit a satisfactory Vulnerable Sector Check, as well as complete any subsequent declaration or check as required by legislation/Act, regulation, and/or policy.
  • Must maintain ability to travel in a timely manner to other offices, work locations or sites as authorized by the Corporation for business reasons.
  • Compliance with any requirements, including immunization and screening measures, of the infection prevention and control program as required by applicable legislation/Act, regulation or policy.
    • Staff are required to be screened for Tuberculosis in accordance with the applicable legislation/Act
