Position Overview

The Registered Dietitian is responsible for providing individualized nutrition and hydration assessments, nutrition risk identification, care planning, monitoring, evaluation and reassessment. This is required to meet the nutritional needs of each Long-Term Care Resident based on both the standards and legislative requirements of the Ministry of Health and Long-Term Care, and the policies and procedures of the County of Lambton. The Registered Dietitian is a member of the interdisciplinary team.

Duties and Responsibilities

Duties:
• Complete an assessment of nutrition status including MDS-RAI (Minimum Data Set-Resident Assessment Instrument) assessments as required; Resident's individualized nutrition care plans are reviewed to meet his or her needs by involving the resident, family members and the care team.
• Evaluate nutrition and hydration status of Resident's food and fluid intake. Provide appropriate follow up for Residents identified at a high nutritional risk.
• Implement interventions to maintain as well as restore the nutritional health of the Residents according to their assessed needs.
• Educate resident, family members, and care givers regarding optimum Resident nutrition care. In conjunction with the Nutrition Supervisor of each home, plan, administer and evaluate staff education needs. Provide ongoing in-services and education programs for staff.
• Participate in the continuous quality improvement of the Nutrition Care and Hydration program in each home.
• Participate in continuous education opportunities. Applies to practice current information regarding food, nutrition, diet and health.
• Document assessments, progress notes, plan of care, reviews and follow-up using Point Click Care software.
• Review and approve all menus including those for therapeutic, texture modified diets and snack menus.
• Consult with and advise other team members, including the Nutrition Supervisors in each home, concerning program needs and plans for dietitian services.

General:
• Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed.
• Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis.

Qualifications

Minimum Formal Education
• Bachelor’s Degree from a recognized university program in Applied Science in Food and Nutrition – 4 year degree
• Completion of a one year accredited Dietetic Internship Program
• Must be a member in good standing of the College of Dietitians of Ontario

Experience
• One year experience is required (can be gained through academic placement)

A valid Ontario Driver’s Licence and use of a vehicle.

Salary & Benefits

Wage Rate: $33.26 - $39.60/Hour
